Online Application Form Builder With Review Steps

An application form gathers answers and documents and sends each one to the person who decides. FormsBuilder keeps the whole path in one place, from the first answer to the recorded decision.

What you can build

Applications that reach the right reviewer

Collect answers, uploads and consent, then route each application to a reviewer with a recorded decision. Common examples include:

  • A job application with CV upload, a short "Why this role" answer and a salary expectation number field
  • A scholarship application with transcripts, a personal statement and a signed declaration
  • A club membership application with references and a signature
  • A grant or program application with budget figures and supporting files

Fields that fit the process

Use text fields for open answers, choice fields for categories, a date field for availability and an address field for applicants. The file upload field takes documents and attachments, and a signature field, drawn by finger or mouse, captures consent. Each signed submission can include a PDF copy. Signatures and approvals are part of the Plus plan at 149 USD per month.

How it works

From submission to decision

Each application is stored with its files in a submission inbox you can search. A reviewer approves or declines, the decision is saved against the submission and the applicant is told the outcome. The same flow is described for online application forms.

Routing with conditional logic

Rules decide where each application goes. For example, "if Department is Engineering, send to the hiring lead for engineering" and "if Age group is Under 18, add a step for guardian consent". Rules can also skip steps that do not apply, such as a portfolio step for roles that do not need one. Conditional logic is included from the Starter plan at 49 USD per month.

Long applications in steps

A long application is easier to finish when it is split into steps with a progress bar: "About you", "Experience", "Documents" and "Consent". Validation catches mistakes on the spot, such as a missing required file or a date outside the allowed range.

Fees and deposits

If your program charges an application fee, add a payment step with a fixed price, or a price calculated from answers, for example a lower fee for students. The application is saved after payment is confirmed, so you never review an application that was not paid for.

Why FormsBuilder

A clear record of every decision

Roles and the audit log on Pro (499 USD per month, 249 USD billed yearly) show who reviewed what and when. Roles let you separate people who may view applications from those who may decide. Unlimited team members mean you can add every reviewer without paying per seat.

Two ways to share the form

Embed the application on your own page with a snippet, or share it as a link in an email or a post. Applicants receive a confirmation, and your team receives a notification when a new application arrives.

Exports for the committee

Selection committees often want a spreadsheet. Export submissions to a spreadsheet file for sorting and scoring, or export individual applications as PDF for the file. Describe your process to the generator for a first draft, then refine it in the editor.
