Online Application Forms With Review Steps
Applications need answers, documents and a decision. Keep all three together so nothing gets lost between an inbox and a spreadsheet.
Use case
Run applications from one place
Applicants complete the form and upload documents. Reviewers see each application, decide and record the outcome. Because the files travel with the answers, nobody has to chase an attachment.
- Document uploads and consent
- Reviewer assignment and decision
- Outcome email to the applicant
See the online application form builder.
A worked example
Imagine a community college running a scholarship round. The form has four steps with a progress bar.
- Step one, "About you": full name, email, phone, date of birth and an address field
- Step two, "Your studies": program of study as a choice field, expected graduation date and a number field for grade average
- Step three, "Documents": file upload for transcripts and a short personal statement
- Step four, "Consent": a signature field, drawn with a finger or mouse, confirming that the information is correct
The rules are simple. If the applicant is under 18, a guardian step appears with a second signature. If the program is "Nursing", the application goes to the nursing coordinator, and any other program goes to the general committee. Conditional logic is available from the Starter plan at 49 USD per month.
Reviewers and decisions
Each reviewer opens the submission, reads the answers, opens the documents and chooses approve or decline. The decision is saved with the submission. If a second reviewer must confirm, add another approval step. Approval steps, signatures and the PDF copy of each signed submission belong to the Plus plan at 149 USD per month.
The applicant side
After sending the form, the applicant receives a confirmation. When the decision is made, they get an outcome email. Your team receives a notification for every new application, so reviewers know when work is waiting.
Who sees what
Not everyone on the team should decide. With roles on the Pro plan, you can let staff view applications while only committee members approve. The audit log on Pro lists who looked at or changed an application, which matters when a candidate asks how a decision was reached.
Fees, deadlines and limits
If you charge an application fee, add a payment step with a fixed price. The application is saved after the payment is confirmed. For deadlines, use a date field to collect availability or a start date, and use validation to reject dates in the past. Plus allows 50 forms and 5,000 submissions a month, which is enough for most admission rounds.
Search and export
All applications sit in a submission inbox with search, so finding "Maria Lopez" or all applications for one program takes seconds. Export to a spreadsheet file for scoring sessions, or to PDF when a file must be archived.
