What you can build

Safeguards for sensitive intake forms

Whether you meet your own legal obligations depends on how your organization collects, stores and shares information. A form tool can provide technical safeguards, and FormsBuilder offers several of them.

  • Encryption of submissions in transit and at rest
  • Role based access so only the right people read submissions (Pro plan)
  • An audit log of changes (Pro plan)
  • SSO (SAML) for your team (Pro plan)
  • Export and deletion of submissions under your control

This page describes safeguards only. It is not a statement that using FormsBuilder makes your organization compliant, and it is not legal advice.

An intake form that collects only what is needed

A good intake form starts with restraint. A physical therapy clinic might ask for name, date of birth, phone, the reason for the visit, insurance member number and a consent signature. It does not need a full medical history on the first screen. Fewer fields mean less sensitive data to protect.

How it works

Building the intake form

Describe the form, review the draft and adjust the fields. Use a multi step layout with a progress bar so a long intake feels manageable: Contact, Reason for visit, Insurance, Consent. Validation keeps required details from being skipped, and the form works on a phone in the waiting room.

Showing only what applies

Conditional logic keeps questions relevant. If the patient answers "Yes" to "Are you a new patient", show the history questions and skip them for returning patients. If the visit type is Telehealth, hide the parking instructions. Conditional logic and multi step forms come with the Starter plan at 49 USD a month (24 USD a month billed yearly).

Consent and signatures

Add a signature field so the person can sign with a finger or a mouse. On the Plus plan at 149 USD a month (74 USD a month billed yearly), the submission also has a PDF copy with the signature, which you can store with your own records.

Controlling who sees what

Roles and the audit log

On the Pro plan at 499 USD a month (249 USD a month billed yearly), roles let you give front desk staff and clinicians different access. The audit log records changes, so you can review who did what. Pro also includes SSO (SAML), white label, invoices and an SLA. Team members are unlimited.

Routing and approvals

A rule can send a submission to a particular person, for example a billing question to the billing coordinator and a clinical question to the nurse. An approval step records a decision with the submission when a staff member must review before the next action.

What stays with you

FormsBuilder does not replace your compliance program. Review your obligations, decide which information you collect, collect only what you need, and train your staff on handling submissions. You control exports and deletions of the data you collect.
